The FA-10 form that was once downloadable here, no longer is. Here's why...

Good afternoon:
My office recently received a complaint from an individual who accessed a PDF version of a firearms transaction form (FA-10) found on the Haverhill Hound Rod and Gun Club website (http://www.hhrg.org/main.html).

Pursuant to G.L. c. 140, § 128A, firearms transactions must be recorded on forms furnished by the Massachusetts Department of Criminal Justice Information Services (DCJIS).  Photocopies and electronic/PDF versions of FA-10 forms are not furnished by DCJIS, are not approved and are not be accepted or processed by the Firearms Records Bureau (FRB). 

DCJIS/FRB has provided residents of the Commonwealth with two convenient options for complying with the requirements of G.L. c. 140, § 128A:   1) The Massachusetts Firearms Registration and Transfer System (E-FA-10), an internet-based application that allows residents of the Commonwealth to safely and securely submit firearms transfer and registration records electronically from any internet enabled computer (available at: https://mircs.chs.state.ma.us/fa10); and  2) sequentially numbered, standardized FA-10 forms (available at all local police departments).

I would like to request that the link to this form be removed from the Haverhill Hound Rod and Gun Club website.  Any forms that are printed off of the website and submitted to the FRB will be automatically returned to the sender.  If you have any questions, you may contact me directly at 617-660-4701.
